Text description provided by the architects. A private residence in Karuizawa, Nagano. Situated along the road lies the distinctive triangular property extending 110m in length. A creative solution was essential in securing privacy from passing traffic and pedestrians while acting as a foil for basking in the natural surroundings.

The approach was to design concrete block walls that would serve as filter screens. The line of sight is orchestrated by adjusting the angles of each block, fine-tuned by overlapping two rows of blocks in some places. In areas with a single row, the view is set in one direction. With two rows overlapping, the view on the other side is visible only when the block angles in the front and back rows align; otherwise, the view is obstructed.

The material used for the blocks is "CO2-SUICOM※," which is made by replacing a portion of cement with an industrial byproduct and adding a carbon dioxide-absorbing material for less CO2 emissions during production.

Approximately 2,050 blocks are staggered in parallel rows to create five 3m-high walls. The living space is designed to "fill in the gaps" in between. Calibrating the block angles by three degrees to fit each room condition ensures privacy in the bedroom and bathroom while also providing a spacious ambiance in the living and dining rooms. Ultimately, the design transpires into blocks arranged in a gradient-like pattern.

Acute appendicitis is one of the most common surgical emergencies worldwide. Many complications can occur secondary to complicated appendicitis including abscess formation, gangrene, sepsis, and perforation, rarely, leading to abdominal wall necrotizing fasciitis. The incidence of necrotizing fasciitis as a complication of ruptured appendicitis is extremely uncommon. The formation of an enterocutaneous fistula leading to this complication further emphasizes the rarity of such occurrence with few cases reported in the literature. Herein, we present a case of abdominal wall necrotizing fasciitis in a 72-year-old female presenting to the local emergency room with complaints of severe suprapubic abdominal pain associated with abdominal distension and acute onset foul-smelling drainage. Physical exam was significant for suprapubic and right lower quadrant abdominal tenderness with associated large indurated tender lesion and purulent weeping with large ecchymosis. Abdominal computed tomography (CT) revealed extensive subcutaneous emphysema, a large cavity with layering fluid extending into the peritoneal space, and a possible fistula formation between the intra-abdominal cavity and subcutaneous tissue. Following the diagnosis of probable necrotizing fasciitis secondary to fistula formation, the patient underwent emergent exploratory laparotomy and extensive debridement of necrotic tissue. In this report, we take the opportunity to highlight the importance of promptly recognizing and treating this uncommon complication and maintaining a high level of suspicion to prevent life-threatening consequences.

Necrotizing Fasciitis of the Abdominal Wall Secondary to Complicated Appendicitis: A Case Report

Osteitis condensans ilii (OCI) is a rare self-limiting low back pain syndrome. It is an incidental imaging discovery around sacroiliac joints with distinctive sclerotic lesions. We present three case reports as a series. In the first case, a 38-year-old female presented with unresolved chronic low back pain for eight years. She had bilateral triangular sclerosis of the ilium abutting sacroiliac joints and other causes of back pain were ruled out. Non-operative management was successful. In the second case, a 38-year-old female presented with acute exacerbation of low back pain for one year. She suffered low back pain during her pregnancy and postpartum period 16 years ago and intermittently after that. Bilateral radiodensity around sacroiliac joints was noted in the radiograph and she had successful remission with non-operative management. In the third case, a 45-year-old female presented with chronic low back for six years. On radiographs, she had bilateral sclerotic lesions around sacroiliac joints and responded well to non-operative management. OCI is a benign, idiopathic cause of axial low back pain. It is a diagnosis of exclusion and the pelvic radiographs classically show areas of sclerosis in the ilium adjacent to sacroiliac joints bilaterally. Treatment for OCI is essentially non-operative.
